Our mission is to provide all-inclusive health and wellness services for older adults that promote quality of life and independence and empower their ability to remain safely in their home and engaged in their community.

One source for all of your care

When you enroll in PACE Central Michigan, we provide all of your necessary care. However, you may be financially responsible for any care you receive outside the program that is not approved by the PACE Central Michigan interdisciplinary team (except in an emergency).

Stay in the home you love, see us for the care you need.

Our goal is to provide our PACE participants with the care, medical treatment and support they need to achieve the highest quality of life – while staying independent as long as possible. Our elder care experts offer an alternative to nursing home placement that features comprehensive, coordinated care for a senior’s medical, social and physical needs, while also providing peace of mind for family caregivers.
